This promotion commences on 30 November 2010 at 2:00pm. Entries close Wednesday 15 December 2010 at 11:55pm.
Access to the customer survey is online from www.csf.com.au following the links to the Catholic Super’s online 2010 Annual Report where the online customer survey will be available. An email will also be sent to active members, who allow us to communicate via email, to complete the survey from within the online 2010 annual report. Entry is open to all completed surveys with an email address or postal address and survey has been done by close of entries date. All valid entries will go into a random drawer where 10 winners will be drawn. All winners will receive a Myer gift voucher valued at $200. The total prize pool of 10 vouchers total $2,000.
The draw will take place on Thursday 16 December 2010 at 10:00am at Catholic Super Head Office located at Level 1/535 Bourke Street, Melbourne Victoria and the winners will be notified by email (if supplied) by Friday 17 December 2010. The names of the winners will be published on the Catholic Super website www.csf.com.au by Monday 20 December 2010.
A draw for the prize if unclaimed will take place on 18 March 2011 at the same time and place as the original draw, Level 1/535 Bourke Street, Melbourne Victoria at 10:00am. Winners name(s) will be published on the Catholic Super website www.csf.com.au by 25 March 2010.
Entry not open to employees (or members of their family) of Catholic Super and its administrator.
