This promotion commences on 30 November 2009 at 2:00pm. Entries close Wednesday 16 December 2009 at 11:55pm.

Access to the customer survey is online from www.csf.com.au following the links to the Catholic Super’s online 2009 Annual Report where the online customer survey will be available. An email will also be sent to active members, who allow us to communicate via email, to complete the survey from www.csf.com.au/customer-survey-nov09. Entry is open to all completed surveys with an email address or postal address and survey has been done by close of entries date. All valid entries will go into a random drawer where 40 winners will be drawn. All winners will receive 2 Adult Village Movie passes valued at $34. The total prize pool of 40 double movie passes total $1,360.

The draw will take place on Thursday 17 December 2009 at 10:00am at Catholic Super Head Office located at Level 1/535 Bourke Street, Melbourne Victoria and the winners will be notified by email by Friday 18 December 2009. The names of the winners will be published in the next issue of the member NOW newsletter in February 2010 and on the Catholic Super website www.csf.com.au by Monday 21 December 2009.

A draw for the prize if unclaimed will take place on 18 March 2010 at the same time and place as the original draw, Level 1/535 Bourke Street, Melbourne Victoria at 10:00am. Winners name(s) will be published in the next issue monthly of the member NOW newsletter and on the Catholic Super website www.csf.com.au by 25 March 2010.
